Record & Accurately Report 

FORM I-693

The results of applicants immigration medical examination and laboratory findings will be recorded and reported on the current version of Form I-693.  *Vaccination record is required for applicants under age 18 but if you are older than 18 and you have your records please bring them.  The fee includes a comprehensive medical evaluation and physical examination, Tuberculosis (TB) Quantiferon blood test, Gonorrhea and Syphilis test and completion of Form I-693.  If your TB test is positive you will need to get a chest x-ray.  Any positive test results (TB, gonorrhea, or syphilis ) will need to be treated as per protocol before we can complete your paperwork.  At the end of the medical, applicant will receive the completed  Immigration I-693 Form in a sealed envelope to provide to USCIS.  Only one  original copy of the I-693 Form is allowed.  Please do not lose or damage the packet given to you.  A copy of the completed application will also be given to the applicant for their record.

Other test required: 

  • October 1, 2018 - Quantiferon TB test for tuberculosis screening (skin test for TB is no longer accepted)

  • August 1, 2016 -  Nucleic Acid Amplification test  for gonorrhea in all applicants 15 years and older.   

 

We provide all vaccinations at the office for a fee, however, you can also obtain vaccinations from you personal physician.  We also work with the NYC Department of Health (NYCDOH), and if you qualify you may obtain your vaccination there for a minimal charge (based on a sliding scale or possibly for free).  Please do not try to meet the vaccination requirement ahead of time.  We will inform you as to what vaccines you require.  We may also direct you to a local pharmacy if desired.
